What Is a Dual Diagnosis?

A dual diagnosis — also known as a co-occurring disorder — is the presence of both a mental illness and a substance use disorder at the same time. This combination is more common than many realize and requires specialized care. Underlying conditions such as anxiety, depression, or PTSD often intensify the cycle of addiction.

According to SAMHSA, millions of adults in the U.S. live with both a substance use disorder and a mental illness. Without integrated treatment for both conditions, individuals are more likely to face repeated crises, hospitalizations, or relapse.

Diagnosing co-occurring disorders takes a careful, thorough approach. Because mental health symptoms and substance use can overlap — for example, withdrawal can look like anxiety or depression — trained professionals begin by gathering a full picture of mental health history, substance use patterns, and current symptoms.

A full assessment often includes clinical interviews, self-report tools, substance use history, functional impairment evaluations, and social determinants such as housing, employment, and support systems. In many cases, a mental health diagnosis is confirmed after detox or a period of stability, when symptoms are easier to evaluate.
