The Clinical Picture We Treat

Substance use in this group rarely shows up by itself. It’s usually connected to years of on-the-job stress, moral injury, poor sleep from shift work, and trauma that’s never been treated. That’s why our assessment looks at the whole pattern — not just one symptom at a time.

Post-Traumatic Stress Disorder (PTSD)

PTSD can show up as flashbacks, avoidance, constant alertness, and negative thoughts after combat, a line-of-duty incident, or repeated exposure to trauma. We treat it using proven therapies — Cognitive Processing Therapy (CPT), Prolonged Exposure (PE), and EMDR — delivered by clinicians trained specifically in military and first-responder trauma.

Alcohol and Prescription Opioid Use

Alcohol is the most common substance used in this group, often normalized as part of unit, station, or precinct culture. Prescription opioid dependence after an injury — whether from combat or the job — is another common path to addiction. We offer medically supervised detox referrals and evidence-based medications like buprenorphine, naltrexone, and Vivitrol when appropriate.

Depression, Suicidal Thoughts, and Moral Injury

Veteran suicide rates are significantly higher than the general adult population — about 44% higher for men and 92% higher for women, after adjusting for age. Firefighters and police officers also face elevated risk. That's why depression screening, safety planning, and moral-injury-focused therapy are part of intake from day one — not something added later.

Traumatic Brain Injury (TBI) Considerations

Mild TBI is common among combat veterans and first responders, and it can make both substance use and mental health treatment more complicated. Our clinicians work with neurology specialists and adjust therapy — in both group and individual sessions — to fit each person's needs.

Our Treatment Model

Care moves through our full continuum of treatment — detox referral, residential placement, Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP), and Virtual IOP — with a veterans-and-first-responders track built alongside our standard programming. Our first-responder Virtual IOP starts at 7:00 AM, designed around shift work, duty rosters, and overnight schedules, so you don’t have to choose between treatment and the job.

Trauma-Informed Core

CPT, PE, and EMDR, delivered by clinicians trained in military and first-responder trauma.

Peer Group Cohort

Small process groups made up only of service members and first responders. What's said in the room stays in the room.

Family Reintegration

Spouses and partners can join our Connections Family Program, both during and after treatment.

Career-Safe Planning

Discharge planning that takes fitness-for-duty evaluations, POST/POAM requirements, and return-to-work timelines into account.

Medication Management

Board-certified psychiatric providers on staff to manage depression, PTSD, anxiety, and medication-assisted treatment (MAT).

Physical Restoration

Sleep, nutrition, and light physical activity — not extras, but part of the clinical plan to help your body and mind recover.

Confidentiality & Fitness-For-Duty Concerns

Many service members and first responders put off treatment because they’re worried about what it could mean for their career. Here’s the truth: Wavecrest follows HIPAA and 42 CFR Part 2, a federal law that gives substance use records even stronger privacy protection than standard medical records. We don’t share the fact that you’re in treatment with your employer, command, or licensing board unless you give us written permission to do so — and you can revoke that permission at any time. If an agency or IME requires a fitness-for-duty evaluation, we work with you and your representative ahead of time to agree on exactly what gets shared.
